By completing the online joining process, you are registering to participate in activities held by Kate Campbell Fitness, and you are accepting this waiver and all the terms and conditions (insofar as they are applicable). You should only join if you have read them and voluntarily accept them.
The participant acknowledges that there are certain risks of injury to themselves or third parties associated with their involvement in personal training sessions, programmes, courses, bootcamps, gym sessions, machines, and classes, whether in-house or online.
You should always consult your doctor before starting any kind of exercise or physical activity programme. This form is a vital legal document. It outlines the risks you accept by beginning an exercise programme. It is essential that you read and fully understand it.
Participants should seek professional advice if they have any known medical conditions or injuries before the session. Any nutritional information is a guide only; if you have any medical conditions or an eating disorder, are underweight, pregnant or breastfeeding, then you will need to discuss with your health care professional before you participate.
All clients are required to complete the relevant screening forms online.
For most people, physical activity should not pose any problems or hazards. Screening Forms have been designed to identify a small number of people for whom physical activity might be inappropriate or those who should have medical advice concerning the type of activity most suitable for them.
When completing the screening form, read it carefully and answer the questions honestly. If you answer yes to any of the questions, more details are required. You may be asked to seek medical advice or approval before taking part. If your health changes, you may be advised to refrain from further exercise until you have consulted with your healthcare professional.
It is essential that you listen to your own body and work at your own pace. Staying hydrated and taking a rest whenever needed is vital. Please inform the instructor of any medical conditions or injuries. If you have any questions, feel unsafe, or become unwell, please inform the instructor immediately and discontinue further participation in the activity.
You are fully aware you are free to withdraw from the activity at anytime and are participating of your own free will. You agree to follow the instructions given by Kate Campbell Fitness trainers and agree to abide by the rules for participating in various activities.
The instructions and advice presented are in no way intended to be a substitute for medical counselling or advice. Not all exercises are suitable for everyone, and this or any other exercise program may result in injury.
Any user of the exercise programs assumes the risk of injury that may result from performing the exercises shown. The creators, producers, participants, instructors and distributors of this program disclaim any liability or loss of connection with the exercises and advice contained herein.
I hereby declare that I wish to participate in the activity, and I understand that doing so involves potential risks of injury, including but not limited to bodily injury, heart attack, stroke, or even death. I also acknowledge that there are other risks associated with exercise and that it is not possible to list every one. I am aware of and accept the risks of injury that could arise from my participation in any session or programme. I declare that I am in good health and capable of taking part in the activities. I accept full responsibility for my own health and wellbeing and agree that any instructions received will be at my own risk.
You agree that in the event you are injured or suffer damage, you will bring no claim, legal or otherwise, against Kate Campbell Fitness, Kate Campbell, its employees, self-employed freelance instructors, personal trainers, agents and representatives.
My acceptance also confirms my consent to being videotaped, audio recorded, and/or photographed during the class, and grants the instructor exclusive rights to own, use, and modify the images and recordings for any purpose worldwide. This includes, but is not limited to, advertisements, marketing, and all commercial uses across all media, without any compensation (including royalties) in perpetuity. I waive any right to inspect or approve the images or recordings, and I relinquish any ownership rights to them.
